Concurrent Disorders/Mental Health Case Management

Concurrent Disorders Case Management is a short-term (up to 4 months) intensive case management program for people who are struggling with both mental health and substance use issues. The Case Manager conducts a strength-based assessment and develops a treatment plan in collaboration with the client to address housing needs and provides support, linkages and referrals to psychiatric services, residential substance use treatment and long-term supports.
Mental Health Case Management is a short-term (up to 4 months) intensive case management program for people with mental health issues who are being released from provincial custody. The Case Manager conducts a strength-based assessment and develops a treatment plan in collaboration with the client to address housing needs and provides support, linkages and referrals to psychiatric services and long-term supports.
Hours
Monday to Friday, daytime and evening appointments are available.
Eligibility Criteria
Age: 16+
Legal Status: Involved with criminal justice system, court or probation referral.
